Our rehabilitation care team is made up of
experienced occupational, speech, and physical
therapists, as well as nurses, social workers,
and certified nursing assistants. Their goals
are the same as yours: focus on health and
wellness activities to maximize independence and
minimize your length of stay. We offer
pre-discharge home evaluations from our staff,
patient and family education and home care
services planning, as well as resident and
family services.

Maywood Center for Health & Rehabilitation has
specialized in delivering quality long term care
to residents for more than 30 years. Upon
admission, each resident is evaluated by our
interdisciplinary team. The team develops a
comprehensive care plan which includes all
medical, nursing, rehabilitation, nutritional,
psychosocial and therapeutic needs. The resident
and family members are invited to participate in
the comprehensive care planning process. Each
care plan is individualized; taking into account
a resident’s likes and dislikes, hobbies,
interests and capacities. |

You never grow tired of
the finer things in life, that’s why we pay so
much attention to the details of gracious
living. The living and entertaining areas at
Maywood are beautifully appointed and warmly
inviting. Residential rooms are tastefully
decorated and spacious enough to accommodate our
resident’s special touches and treasures from
home.

“institutional” about the dining at Maywood. Our
chef presents continental cuisine that is
nutritionally correct, but moreover, beautiful

Our
Social Services department, staffed with
concerned and dedicated professionals, helps
residents adjust to becoming members of their
new community. They work with the resident on a
one-to-one basis and serve as a liaison between
staff and families. They are here to answer
questions and offer support and guidance.
Family
involvement is an important component of each
resident’s plan of care. Family members are
invited to attend care plan meetings and be an
active participant in the care of their loved
one. Along with our interdisciplinary team of
professionals, together we shall strive to
improve the residents’ quality of life
throughout their stay. |
